Breast Health and Beyond: A Provider’s Guide to the Examination and Screening of Women with Disabilities 

Continuing Medical Education Credit

This is an activity offered by Alta Bates Summit Medical Center, a CMA-accredited provider.   Physicians completing this course may report up to 4 hours ofCategory 1 credit towards the California Medical Association’s Certificate of Continuing Medical Education and the American Medical Association’s Physician’s Recognition Award.

The California Board of Registered Nursing has recognized CMA Category 1 credit as acceptable for equivalent CE credit towards nursing license renewal.

Alta Bates Medical Center has been granted Board of Behavioral Sciences approval to offer continuing education for MFCC and/or LCSW credit. The provider approval number is PCE 637.

If you would like to receive continuing education credit for this course please:

1. Complete the home study course.
2. Answer all the post-test questions and review the answer sheet.

Additional Information:

• Please do not print, publish, copy or distribute any parts of BHAWD’s Provider Guide including the evaluation form.
• This home study course can not be repeated for the 4 hours of continuing education credits at any time.
• You can mail or fax back the completed evaluation form to BHAWD.
• Once you return your completed evaluation form to us, we will issue your certificate of completion within 14 business days of receipt of the evaluation form.
• The postmark/or fax date of your completed evaluation form determines the date that is issued on your certificate of completion.

• There is no charge for ABSMC Providers.
• Price: $70 (USD) per home study course.
• The price of the home study course includes one Provider Guide, one post-test with answer sheet, one evaluation form, and shipping through USPS.
• Additional post test with answer sheet and evaluation form may be purchased for an additional $55 (USD). 
• The cost of the Provider Guide is $15 each.
